Health Science Program Evaluation Form
Before submitting this form the following are required:
- Admission Form to College of DuPage with $20 Admission Fee (If you have taken credit courses at College of DuPage, you have already completed this step.)
- Application to the Health Science Program with $50 Admission Fee
This Health Science Program Evaluation Form is a formal request to have your credits
evaluated. The evaluation will include your College of DuPage credits along with any
transfer credits from other colleges and prerequisites from High School. The results
of the evaluation will be sent to the Health Sciences Admissions office. Complete
and return this form to the Office of Student Records after your official transcripts
have been received from previous institutions.
IMPORTANT: You can verify online if your transcripts have been received by the Office of Student Records by going to myaccess.cod.edu. Login to your account and select Student Profile.
