Citation must be appealed within 5 days of the date of issuance

This form must be completed by appellant.

The Appeals Committee will review your appeal. You will be notified with disposition of appeal. If your appeal is not granted, you will be given 10 days to pay the fine. If the fine is not paid, a hold will be placed on your account and you will be unable to register.
