If you are currently enrolled at College of DuPage or if you were enrolled and you need to submit a recommendation letter or form to transfer to another institution, you may need to provide proof of your academic standing, disciplinary record or a personal reference as part of your application.
If you need a letter of recommendation from an instructor/counselor, you will need to:
- Contact your instructor or counselor and ask them to write a recommendation letter and complete the Letter of Recommendation Authorization. Your instructor or counselor should submit a copy of the authorization form to the Office of Student Records in SRC 2150.
If the institution that you are applying to has a form to fill out that includes a College Official's Report, Transfer Form or Dean's Recommendation for Transfer Students, you will need to either:
- Visit the Office of Student Records in the Student Resource Centrer (SRC) 2150
- Email a formal request to firstname.lastname@example.org or
- Follow the Enrollment Verification instructions to complete form. Please provide the form as an attachment and send it to email@example.com.
Forms will be processed in the order received and could take up to five business days to complete. Please submit your form(s) in a timely manner. The Office of Student Records will contact you at the email address provided once your request has been completed.
